    DO NOT send your loved ones here! My mother was admitted after a stay at an acute care hospital. There was one CNA and one RN for 28 patients. My mother did not get her night meds until after 11pm and didn't even get all of them. She was waiting for well over an hour for anyone to help get change after an accident when she and my Dad who was there with her asked for help. My Dad ended up staying all night to make sure she was safe and then we removed her first thing in the morning. The facility had zero concerns about us taking my mother out within 18 hours of admission. I'm filling a complaint with state licensing and Medicare Monday morning. Do not use this facility if your loved one actually needs care.

    I have so much to say about this facility. I'm very impressed with this facility as well as the healthcare workers. And thanks to front desk workers, activities people, all the PT and OT people, the Maintenace crew, the janitors and the cafeteria. I appreciate all the different meals they came up with. 5 stars to all of you. Being here for 2 months, I can not say one bad thing about Springdale. I kept communications open and any concerns I had or my partner, these problems were taken care of. All our families are living out of AZ and Springdale is our family. My partner will be here until she goes to her home, where no more pain will hurt her. The facility has much love and caring - I had bad days and they were there to comfort me. Janice made sure I had water and was a great sport answering my call light. She is a hard worker and I give her 5 stars. All the nursing were great too. I give them 5 stars. I will be giving Springdale 5 stars. Everyone deserves a thank you from me and Addie. so Thank You! Bless all of you and keep doing the best you can do. Always remember the smiles and laughter in room 37! - Donna

    My mom stayed here for 4 nights after a recent hospital stay. The building is older, although I think they may be renovating it. Her room was very warm the whole time and they didn't/couldn't adjust the temperature. I ended up bringing her a small fan to try to help. The nursing staff is kind, but many of them seem to be a bit incompetent. She's on oxygen, and needs to wear a bipap at night. The nurses that were there for the first two nights didn't know how to use it, and even when I tried to show one of them, she shook her head and was unwilling to try. She didn't get her meds until the evening of the second day she was there, and the schedule for giving meds was inconsistent. The physical therapist she saw was wonderful. The food was just OK, but she didn't really get to choose anything from a menu. One woman came around on Sunday and took her order for Monday, but the rest of the time she just got whatever they gave her. I tried calling the front desk to arrange transportation for a doctor's appointment, but the woman said to call back later. When I did, I left a voicemail, but nobody ever got back to me. I ended up taking the day off to take her myself. I was able to get her discharged after 4 nights because I just didn't feel confident that her needs were being met there. When I arrived to pick her up, several people rushed in to get her to sign some documents that were supposed to have been signed upon her arrival. Looking at older reviews, I think that this place was probably great in the past, but the current management could benefit from tightening up some things with staffing/training.

    I wouldn’t send my dog here. I spent a week here and it was a nightmare. I had a broken femur and the nurse forgot to order my medication? I was told I didn’t have to cover my Picc line before I shower by a nurse who wouldn’t help. When I was dropped off here from the Mayo Clinic the van driver dropped my papers from the Mayo Clinic to the receptionist then he took me to the room number we were told. He put me in bed and left wishing me luck! Nobody ever came to see me!!! It got dark quickly so I laid in the dark with a broken femur and no way of getting out of bed. I finally called the front desk with my cell phone that luckily had a battery life of 10%. When the lady answered she said they forgot about me. She apologized and gave me food that was in a chair for hours and said it was my dinner. I was so hungry I ate it with my fingers. To make a long story shorter I complained to the director after asking to speak to him for three days. I told him I filed a complaint with the State licensing Board and he whipped out a shiny badge and said he didn’t care because he was the president. I found out later he shouldn’t have had that badge because they stripped him of it previously. He then sent me home with no IV for my infection that same day because he was mad at me. You can’t make this stuff up! Worst week of my life. If my infection comes back I will sue him and Springdale for negligence. I am not a vindictive type of person but he shouldn’t be in this line of business. PS. The poor old ladies just cried all day because no one would help them go to the bathroom. They looked to be pretty drugged up too. The first night there they couldn’t find the emergency cart and someone died.

    Springdale Nursing Care Facility has the worst nursing staff and care I have ever experienced. My mom was at this facility for weeks and her final days of life. She was under hospice care for the last 2 days to make her passing peaceful and comfortable. I was by her side and couldn’t get the medication administered when needed and found out the morning of her passing that she was not given all of the medications she should have been receiving. Do not trust this place with your loved ones!
