    In response to Ninve Smith: We understand how difficult it can be to see a loved one in memory care, please know that we take every report seriously. Both the Arizona Department of Health Services and Adult Protective Services conducted thorough investigations into these claims and found them to be UNSUBSTANTIATED. The 30-day notice referenced was not issued in retaliation but rather due to a deterioration of the family relationship, which made it difficult to continue care collaboratively. Our priority remains the safety, dignity, and well-being of all residents, and our team is committed to providing compassionate, professional care in a safe environment.

    Our parents lived at Vineyard Park for two years. During that time, everyone employed at the Community (Caregivers, Administrative Staff, Maintenance Staff, Kitchen Staff, Front Desk) were engaging, friendly, respectful, compassionate and professional without exception. The Community is extremely well maintained and clean, the dining room delivered consistent and well-executed meals, and the communication between staff, residents and families is exceptional. My parents enjoyed their bi-weekly visit to the Beauty Salon, the musicians who performed weekly, the safety of the environment in which they lived and their constant engagement with staff. We want to personally thank each and every member of the Vineyard Park team for the care and friendship they provided my parents during their time at this beautiful community.

    This was such a great place for my Grandmother, the people here really care about the residents; whether it’s actual residents themselves or family of residents. She made friends quickly with some ladies here and had a blast, up until the day she had her stroke her and the ladies were “running the show”; she was showing an extra spark of life. This place is special and I’d recommend it to everyone, God bless the staff and the facilities staff.

    My mother has been a resident in the memory care unit at Vineyard of Surprise since the end of March this year. From the beginning, I repeatedly raised concerns about finding her prescribed pills on the floor, but nothing was done until I reported the matter to her case manager, who then filed a complaint with the state. Following this, I was told they would create a care plan to prevent further issues. Shortly afterward, Director Korina Izaguirre called me in an aggressive manner, claiming the complaint alleged that facility was not giving my mother her pills—an inaccurate and misleading statement. I explained that I had reported finding multiple pills on the floor multiple times, which meant my mother was not receiving them properly. My mother has also experienced numerous falls, sometimes remaining on the floor for over five hours before staff found her sometimes. This was raised to Korina Izaguirre multiple times. Additionally, residents frequently wander into her room, sometimes getting into her bed. Most concerning, a male resident has repeatedly entered her room and engaged in inappropriate and unsanitary behavior, including urinating in her closet and in her trash can. My mother, who is unable to speak due to a stroke and has aphasia, has been visibly frightened by these incidents (which I have on camera) . Other residents, including one with a dog (which my mother fears), have also entered her room. After I emailed Korina to request a solution—particularly to keep the male resident away from my mother’s room—she told me in person she “didn’t know how to respond” because “it’s memory care,” then never followed up. When my sister emailed again, Korina scheduled a meeting with us and the case manager, where she was again aggressive. We expressed our concern for my mother’s safety and for the safety of other residents, as the male resident wanders into other rooms as well. Korina ultimately stated that since she could not meet our “requirements” of keeping my mother safe, she was issuing a 30-day notice for her to leave, which I feel is retaliation for voicing my concerns. Basic safety and sanitation are not unreasonable expectations, and Korina’s dismissive and unprofessional responses—especially regarding a male resident exposing himself in front of others—are unacceptable.

    Choosing the right place for your family at this stage of life is beyond difficult. Korina and Colette made these decisions so much easier. Living so far away, we placed a great deal of trust in the entire team at Vineyard Park. Each person rose to the occasion every day. They ensured care, respect and dignity for our family during every step of this journey. We appreciate all they have done for us and would recommend their care and guidance to all in this same stage of life. We can never thank them enough for being there when we could not. ❤️
